How to Register
If you live in our practice area and would like to register with us please bring your medical card to the practice and those of any other persons registering with you.
If you do not have a medical card please complete one of our registration forms that are available from reception or can be completed via the link below.
You will be required to have a new-patient health check with the nurse before your registration is complete.
It is important to remember that you will be registering with the practice rather than an individual GP, although when booking an appointment you can express a preference of a practitioner.

After you have completed the form you must come in to the surgery within 7 days with 2 forms of identification (ID).
Acceptable Identification; tenancy agreement, mortgage agreement, bank statement, driving licence, passport, birth certificate etc.
Until you have submitted your ID you will not be registered at the practice.
